- Applicant pays $100 enrollment fee (non-refundable) with application.
- All charges for care are set up for payment in advance on electronic bank draft from a checking or savings account to be withdrawn on the 3rd of each month.
- An insufficient funds fee (NSF) will be assessed for return drafts ($35).
